Richard E. Sall’s Straightjacket follows thirty days in the life of Dr. Joe Grady, a resident surgeon at Detroit General Hospital, who is obliged to complete ten major surgical cases within this time frame in order to gain his residency certification.

This is the punishment meted out by Dr. Frank Rizzo, also known as “little Hitler” to many of his colleagues, and the hospital’s chief surgeon, as a result of Grady refusing to go along with a request made by the hospital’s chief administrator, Arthur Williams, to give pain medication to a patient who was a known drug seeker. Apparently, one of Gray’s colleagues had initially refused to prescribe medication after discovering during a statewide pharmacy research that the patient had 3600 hydrocodone tablets filled within a three-month period.

The patient threatened to expose his alleged ill-treatment to the press and this ruffles Williams’ feathers for fear of the bad publicity this may engender. Consequently, Williams summoned Grady to his office and requested that he participate in bending the rules and giving the patient what he wants or as he explains to him, “the hospital is like McDonalds-whatever the patient wants, you give it to him.” However, as we discover, Williams ulterior motive was not exactly for humanitarian reasons, for the bad press could harm his hospital’s fund raising campaign.

Straightjacket presents a candid depiction of life in a big city hospital that will surely resonate with any reader who may have spent some time as either a patient or doctor in a hospital. We also receive a poignant picture of the exhausting medical training physicians endure often extending over a period of seven to twelve years of post-bachelor-degree training.

Sall’s writing style is occasionally jarring as we are given front row seats to the surgical procedures that Gray must perform during his remaining thirty days of residency. Romantic tension is thrown into the mix when Grady falls in love with the beautiful Nurse Linda Marie Jablonski, whose mother suffers from Borderline Personality Disorder. To further complicate matters, Linda’s mother threatens to hire a member of the mob to knock him off if he does not stay away from her daughter. The cast of characters are easy to relate to and the insertion of black humor is at times hilarious, particularly with some of Grady’s dialogue and musings.

The challenge a physician or any member of the liberal professions face when writing a work of fiction is to avoid the trap of “déformation professionelle.” This can be loosely translated as when the craft of one’s profession thwarts decision making. In this case, Sall, who is a practicing physician, doesn’t quite strike the right balance between accuracy and comprehensibility, between too much and too little information. As a result, his overuse of medical jargon slows down the pace and flow of the story.

Nonetheless, Straightjacket succeeds as a good read enhanced by a frank look at the deficiencies of the American health system. In other words, although it may be a work of fiction, this book has a great deal more than it seems to be.

Running a pay-per-click advertising campaign can be very expensive if picking the wrong keywords. I was setting up a new PPC campaign the other day and were struggling to find the right keywords. I was promoting an affiliate program in this case. To convert the click (on my ad) to a sale the user had to submit his/her zip code only. Thinking of who might be interested in winning a free digital camera I started evaluating keywords around digital cameras. People looking to buy a new digital camera could be the right target group but to my surprise some common keywords did not generate much traffic at all or were so expensive that there was no profit to make for me. So, now in need of finding keywords that would generate traffic I started evaluating keywords away from digital cameras. What else would you use when having a digital camera?

Well, people still like to print photos and so I started experimenting with keywords around photo printers, ink, cartridges and printer related accessories. You will be surprised how many keywords I suddenly had at hand and how low bidding prices were. Still at the same time the keywords were good enough to create several thousand impressions of my ads a day and to create a good number of clicks that converted to sales.

So, if you are designing a PPC advertising campaign make sure to think outside of the box. If keywords for the actual product are to expensive or do not convert really well, a look at things used around the product might be well worth the time. In my case I was able to convert about 80% of the clicks to a sale. Part of that success was having a great mix of keywords and low bidding prices.

Direct-View televisions have amazing picture quality, while still being less expensive than flat panel types. Screens come as small as 9 inches, and as big as 40 inchesimagine how fun it would be to play the Playstation 3 on that! Because of the shape of the screen, you have a good view from nearly any angle in the room. Unfortunately, many people say that it doesn’t give quite the “immersive” experience that you can get from other types of televisions.

Rear Projection televisions also give excellent image quality, and can be considered to be better than some of the newer technologies. It has the richest and subtlest blacks, and are very very affordable. Unfortunately, they’re bulky. Some models are 30 inches deep, making it difficult to put it into a standard TV cabinet. (And because of its weight, it’s hard to move around.) And they require to be finetuned by an expert to maintain optimum image quality. Some complain that the image starts to dim when viewed from certain angles.

Microdisplays are smaller rear projection TV’s. They have very sharp images, and have no rainbow effect. But it’s difficult to watch comfortably from a short distance (some people describe it as something like watching TV through a screen door), so you need a big viewing room.

DLP rear projections have good black levels, and smooth pictures, but there’s some “noise”sparkles in some areas of the screenand can occasionally have rainbow effects, especially when you move your eyes quickly.

LCO’s have excellent resolutions, some as high as 1080×1920. Prices are high, too, and you may have some problems getting the really rich black colors.

Flat Panel TVs, the new big thing to hit TV, are being praised not only for their high resolution but their form factor. They can sit elegantly in a living room, almost as sleek as a sculpture, and the handy shape also means that they don’t take up valuable space (ideal for those in small city apartments). Disadvantages include burning in of static images. LCD Flat panels don’t have the burn in problem, but may be difficult to view at wide angles.

As you can probably see, choosing the right tTV for you involves many different factors. These include, of course, your budget. How much are you willing to spend on your television? Of course, prices are affected not only by the type of television, but the size. Also look at the set-up of your room. Certain types of televisions are best viewed from some angles, or have blind spots. Where is your sofa located? Where will people be seated when they watch? Consider that when you choose a model.
